In Compassion-Focused Therapy (CFT), developed by Paul Gilbert, compassion is defined as “a sensitivity to the suffering of self and others, with a commitment to try to alleviate and prevent it.” Many clients, however, face significant “fears of compassion” when it is directed toward themselves. The CFT Compassionate Self on Others Worksheet offers a strategic clinical bridge by focusing on the flow of compassion from self to others.

By shifting the focus to a loved one, clients can more easily activate the Soothing System without the immediate interference of self-judgment. This worksheet guides the client to visualize kind words and the recipient’s positive reactions, which strengthens the neural circuits associated with mentalizing and pro-social behavior. This “outsourced” compassion provides a tangible template for what warmth and support look and feel like.

For many individuals—particularly those with a “blocked” or “resistant” relationship with self-compassion—it is often easier to access feelings of warmth and kindness for a loved one first. This worksheet serves as a “backdoor” to the compassionate mind, using external relationships as a training ground for internal emotional regulation.

The Clinical Path to Relational Warmth

This exercise guides the client through a two-stage process of physiological slowing and cognitive empathy:

  • Somatic Priming: One minute of rhythmic breathing lowers the “threat noise,” allowing the client to transition into a state where affiliative emotions are neurologically accessible.
  • Selecting a Compassionate Target: By identifying someone they naturally care for, clients bypass the “inner critic” that often sabotages self-directed kindness.
  • The Empathy Loop: Documenting both the Kind Words and the Person’s Reaction helps clients visualize the reciprocal nature of compassion. It trains the brain to recognize the specific facial expressions and emotional tones associated with safety and warmth.

Strategic Utility in Practice

Clinicians utilize this worksheet to help clients “find the feeling” of compassion. Once the client has successfully activated this state by focusing on another, the therapist can then gently guide them to “turn that same warmth inward.” It is a vital tool for treating attachment-based trauma and interpersonal difficulties.
