The process begins with situational and trigger identification. Clients are encouraged to differentiate between the external situation (what objectively happened) and the internal or contextual trigger (e.g., a physical sensation, a specific thought, or an interpersonal cue). This distinction is clinically important, as many emotional reactions are driven more by internal triggers than by the situation itself.

Next, clients record initial emotional intensity, providing a measurable starting point. This is paired with the identification of unhelpful thoughts, along with a belief rating. These belief ratings are essential—they reveal how convincing the thought feels in the moment, which often predicts the strength of the emotional response.

The worksheet then broadens its focus to include behavioral output, capturing both what the client did and what they avoided. Avoidance is particularly important in CBT formulations, as it frequently maintains anxiety and prevents corrective learning.

From there, the intervention shifts into cognitive restructuring. Clients generate an alternative response that is more balanced, evidence-based, and contextually grounded. This is followed by a second belief rating, allowing comparison between the original and revised interpretations.
