Our Comprehensive BIRP Note Template gives mental health clinicians and practices a complete, professionally designed BIRP progress note — available as both an editable Word document and a fillable PDF that can be customized to your practice setting before use.
Our BIRP Progress Note Template covers all four BIRP sections in full clinical depth — including the full 14-component Mental Status Examination, structured risk assessment matrix, DSM-5-TR diagnosis with ICD-10-CM code, standardized assessment results table, treatment modality and intervention checkboxes, client response documentation, progress-since-last-session goal tracking, and a clinician signature block with supervisor countersignature line. Every element a clinician needs to produce a complete, audit-ready BIRP progress note is built into the structure — so nothing is overlooked under the time pressure of a busy clinical day.
For clinicians in behavioral health agencies, community mental health centers, substance use treatment programs, and managed care settings where BIRP documentation is required or preferred, our template provides the structured, intervention-specific documentation format those settings demand — in a format that is practical enough to use at every session.

BIRP notes — organized into Behavior, Intervention, Response, and Plan sections — are one of the most widely used progress note formats in behavioral health, community mental health, and substance use treatment settings. Our Comprehensive BIRP Note Template gives clinicians a structured, professionally designed document that covers every element of a complete session note, from behavioral observations and the Mental Status Examination through specific intervention documentation, client response, risk assessment, diagnosis, and next steps — all in a consistent format that supports clinical accuracy, insurance documentation, and continuity of care.
Available as both an editable Word document and a fillable PDF, our BIRP Progress Note Template can be customized to your practice setting before use.
What Is Included in Our BIRP Note Template
Our comprehensive BIRP note template covers all four sections in full clinical depth:
- Behavior section — session overview with age, sex or gender, pronouns, reason for session, and distress rating; DSM-5-TR diagnosis with ICD-10-CM code, specifier, and diagnosis changes notation; behavioral observations covering session participation, engagement, and functioning outside sessions; full 14-component Mental Status Examination with descriptor options; standardized assessment results table (PHQ-9, GAD-7, PCL-5); current symptoms and functional impairment; current stressors; symptom review checklist; medical, psychiatric, and substance use updates; structured risk assessment matrix covering SI, HI, self-harm, substance use risk, and other safety concerns with current status, historical status, and risk level ratings; protective factors; risk mitigation actions; and treatment goals addressed this session
- Intervention section — treatment modality checkboxes (CBT, DBT, ACT, EMDR, Motivational Interviewing, and others); specific interventions provided this session; primary focus of session; intervention narrative with clinical rationale; and education and psychoeducation provided
- Response section — client response checkboxes (engaged and receptive, demonstrated insight, practiced skill successfully, resistant/avoidant, ambivalent); additional response notes; progress since last session by treatment goal (Improved / Stable / Worse); clinical observations covering insight gained, skills practiced, motivation, and remaining barriers; and session narrative
- Plan section — planned interventions for future sessions; homework and between-session tasks; medical necessity and continued need for treatment; referrals and coordination of care; next appointment; final notes; and clinician signature with supervisor countersignature line and documentation completion tracking
How Our BIRP Progress Note Template Is Used in Practice
Our BIRP note template is designed for use at every scheduled therapy session as the primary session documentation tool. It is structured to satisfy the documentation requirements that matter most in behavioral health and outpatient mental health practice:
- Routine session documentation — the comprehensive structure provides a complete clinical record of each session that accurately reflects the services rendered, the client’s behavioral presentation, and the clinician’s specific interventions and clinical reasoning
- Insurance authorization and audit support — the dedicated Intervention and Response sections explicitly document the specific therapeutic techniques used and the client’s response to them — the elements managed care organizations and Medicaid programs review most closely during authorization requests and claims audits
- Community mental health and agency settings — the format aligns with the documentation standards commonly required by behavioral health agencies, publicly funded programs, and managed care organizations that prefer BIRP documentation for its explicit intervention and response structure
- Clinical supervision — for training clinics and multi-clinician practices, the consistent format allows supervisors to evaluate intervention documentation, treatment fidelity, and clinical reasoning efficiently across supervisees
Practice Benefits
- Covers every element of a clinically complete BIRP progress note in a single structured document — reducing the risk that critical elements are omitted under time pressure
- Dedicated Intervention and Response sections explicitly document specific therapeutic techniques and client response — the documentation elements most frequently reviewed in managed care and Medicaid audits
- Full 14-component MSE and structured risk assessment matrix bring comprehensive clinical documentation standards to a format that is practical for session-by-session use
- Editable Word document format allows practices to add their name and customize fields to reflect their specific clinical context and documentation requirements
- Fillable PDF format allows clinicians to complete notes digitally on any device — well suited for telehealth practice and for clinicians who prefer digital completion before uploading the finished note to their clinical record
A Note on Clinical Judgment
A BIRP note captures the clinical picture at a single session in time. A client’s behavioral presentation, symptom severity, and response to interventions can shift significantly between sessions and in response to circumstances outside the therapy room. Clinical judgment should always inform how session-level observations are interpreted within the broader treatment trajectory — and cultural competence should guide how clinicians document behavioral presentations and clinical observations without pathologizing cultural differences in expression, communication style, or engagement with treatment.
