In Acceptance and Commitment Therapy (ACT), the goal is not to eliminate these thoughts, but to change how clients relate to them. Cognitive defusion is the process that makes this possible—helping clients step back from thoughts rather than becoming caught up in them.
ACT was developed by Steven C. Hayes, Kelly G. Wilson, and Kirk D. Strosahl, with psychological flexibility as its central aim. Cognitive defusion plays a key role in this model by weakening the literal impact of thoughts. When clients learn to see thoughts as mental events rather than facts, those thoughts tend to lose their ability to dictate behavior or emotional responses.
The ACT Cognitive Defusion Worksheet is designed to make this shift concrete. It walks clients through a step-by-step process of observing a difficult thought, then gradually creating distance from it using structured language and reflection. This worksheet focuses on a specific and teachable defusion sequence. Clients begin by identifying a concise self-critical or distressing thought, then engaging with it in progressively different ways.
The structure is intentional. First, the thought is repeated as-is, allowing clients to notice its immediate emotional impact. Then, distance is introduced by adding the phrase “I’m having the thought that…”. This subtle shift changes the function of the thought—it becomes something observed rather than something automatically believed.
A further step—“I notice that I’m having the thought that…”—creates additional psychological distance. At this stage, clients are no longer inside the thought; they are observing the process of thinking itself. This is a core element of cognitive defusion and directly supports the development of psychological flexibility.
The final reflection component helps clients consolidate what changed across these steps. Rather than relying on explanation alone, the worksheet emphasizes lived experience—what it actually felt like to move from fusion to defusion. This is where the learning becomes durable and transferable to real-world situations.
In Practice
Clinicians often use this worksheet early when introducing defusion because the shifts are simple, observable, and immediate. It can be completed in-session to demonstrate the process, then revisited between sessions to reinforce the skill.
